Once you have made an appointment with one of our specialists you are invited to complete a Client Information Form. If your appointment is with our Dermatologist, Dr. Kathleen Kalaher, you can also complete the Dermatology Information Form. If your appointment is with Dr. Petra Lackner, our Ophthalmologist, you should complete the Ophthalmology Information Form as well. . If your appointment is with one of our internists, Dr. DeMarco, Dr. Pintar or Dr. Worwag and is regarding Diabetes, you should complete the Diabetes Form. All of these forms can be either (1) e-mailed to the hospital or (2) printed and completed at home, then brought with you to the hospital appointment. These forms are provided to our clients as a convenience to save time during the check-in process. Please submit these forms no less than 48 hours before your appointment. Thank You.
